Club Operating Procedures require each club-approved CFI to provide a member with an endorsement whenever said member successfully completes a checkout.
The endorsement helps the club satisfy insurance requirements. In particular, the endorsement is the written evidence the Club would show the insurance company if ever there were a need to prove that a checkout had been completed.
The form below satisfies the endorsement requirement. Print it and bring it with you to your check ride. After you have completed your check ride, ask your instructor to fill out the form and endorse it. Then leave the completed, endorsed form in the club locker or mail it to the Club's PO box.
The Club must have your completed endorsed form in its possession before you solo the airplane. Leaving the completed form in the club locker meets this requirement.
