WHERE: We will meet at St. Thomas and carpool down to Whitewater Canoe Rental in Brookville, IN

FOR: All middle school and high school youth group members and their families and any other interested St. Thomas parishioners.

COST: $10.00 per person

In the event of an emergency where medical treatment is required, I give permission to the church staff, sponsor or volunteer to obtain the services of a licensed physician. Please attempt to notify me immediately concerning any such emergency. Neither the Archdiocese of Indianapolis, St. Thomas Parish, nor any representative of St. Thomas Parish shall be held financially responsible for any injury, illness, or death incurred as a direct or indirect result of this activity. I understand this is an opportunity for spiritual growth. Any behavior contrary to that purpose will result in immediate dismissal and the parents will be called to pick up their child. Also, parents are responsible for making restitution for any damages resulting from the inappropriate behavior of their child.
